This service is for individuals that have a physician's order and using insurance to cover the lab test. If insurance information is not on the lab requisition or physician order, please upload front and back of insurance cards in insurance section of this page or have it handy to show the technician upon arrival. Results will go directly to the ordering physician. If you have test that need to go to more than one lab, please add them in the add-on section of this page.
